Job Description Overview The Social Work & Bereavement Manager oversees the daily operations of the Social Work & Bereavement Departments for all Centra Hospice business units and is responsible for leading and directing the work of both the Social Work & Bereavement teams.
This includes scheduling, providing supervision, coordination and evaluation of Social Worker services in the delivery of care for patients and families with socio-economic, psychosocial, community resource and anticipatory grief and bereavement needs.
Addressing grief and loss needs of Hospice patients and families, staff and volunteers and the bereavement needs of the community at large.
Is responsible/accountable for meeting the requirements of the Medicare Conditions of Participation related to grief and bereavement and the supervision and education of volunteers and staff participating in assessing and reporting the bereavement/grief needs of Hospice families.
The Social Work/Bereavement Manager develops and implements programs to meet the social and emotional needs of patients and families in the home and residential hospice setting.
The Social Work/Bereavement Manager orients, educates, and mentors Social Worker & Bereavement staff and other members of the Home Health and Hospice staff in the promotion of best practice for optimum customer results resource and grief and bereavement needs.
Qualifications/Education/Licensure: Master’s Degree in Social Work from a CSWE accredited program.
Three years recent (within the past five years) experience in acute, long-term care or home care/hospice, with at least one in hospice or homecare preferred.
Licensed as a Social Worker through the Virginia Board of Social Work or eligible for Licensure in state of Virginia.
