Licensed Practical Nurse (LPN)

Cedar Park Med is hiring for Licensed Practical Nurses for immediate openings.
Case Management experience required! *DESCRIPTION: * Responsible for providing services to individuals with mental illness(es) and/or substance use disorders.
Emphasis will be placed on case management services for the SMI, SED, and ATR populations.
This includes clinical assessment, treatment planning, discharge planning, linking with community services, aggressive outreach and follow-up.
The position is exempt for FLSA.
The position is designated as a Level Two with regard to the HIPAA Privacy Rule.
OSHA regulations apply to this position.
Requirements * A bachelors level degree in the human service or related field is preferred.
* QMHP Adult or Child preferred.
* Ability to obtain QMHP-Trainee required.
* Extensive knowledge and experience in assessment, supportive counseling, linking, monitoring and provision of case management treatment services to mentally ill and chemically dependent individuals is preferred.
* A valid Virginia driver's license is required.
Job Types: Full-time, Contract Pay: $43.
00 - $50.
